IMPORTANT PATIENT INFORMATION AND PRACTICE POLICIES
Notice that Services Are Not Primary Care: Please note that Dr. Lund does not act as your primary care provider and does not provide after-hours or urgent care services. The services provided by Dr. Lund are in addition to, and not a replacement for, the care of your primary care provider and other specialists. Dr. Lund’s practice is also a telemedicine practice. Responsibility for your overall medical care should remain with your primary care physician. If you do not have a primary care physician, we strongly recommend that you locate one for in-person examinations and any routine services and screenings that Dr. Lund does not provide, as well as for after-hours or urgent health needs, on-call physician access and emergencies. Dr. Lund is happy to work with your primary care physician and other specialists if desired and encourages collaborative care.
Services and Fees
- Fees are for appointments only and subject to change. They do not include the cost of supplements or lab testing. Lab interpretation and review requires a scheduled visit with Dr. Lund at the normal appointment rates.
- There are no refunds for services already provided.
- Initial Intake Consultation: In-person or HIPAA compliant video visit with Dr. Lund including detailed history, review of records and prior lab testing and assessment of barriers to health. A personalized written plan will be provided which may include preliminary testing, nutritional and lifestyle recommendations, and supplement and/or medication recommendations. Up to 75 minutes is allotted for this visit. A deposit of $295 is required to schedule an initial appointment and the remainder is charged at the conclusion of your visit.
- Lab Review: In-person or HIPAA compliant video visit with Dr. Lund including review of lab results and creation of long-term plan, including your personalized transformation milestones. Interpretation of lab results received by Dr. Lund prior to the scheduled visit. Personalized written plan of care for your health transformation. Generally 30-60 min are allotted for this visit depending on the number or labs done or per practitioner discretion.
- Follow-Up: Visit with Dr. Lund to review your progress and support your continued progress toward your health goals. Personalized written plan of care, modified as needed. Additional testing will be recommended or reviewed as appropriate to help you achieve your targeted health outcomes. Generally 30-60 min is allotted for this visit.
- Phone Check-In: A quick phone call with Dr. Lund. Available for established patients only. 15-30 min is allotted for this call.
We require a credit card on file to hold your scheduled appointments. Please complete the credit card authorization provided to hold your visit and for charges as stated herein. Your credit card on file will be charged for your appointment fee after your appointments are over unless another valid form of payment is provided and in the case of a late cancellation or missed appointment in accordance with our Cancellation, Rescheduling and Missed Appointment Policy set forth below.
Payment
The balance of your appointment fee is due at the time of service. We accept credit cards, debit cards and cash (cash only for in-person services).
- Your credit card on file will be charged at the conclusion of your appointment unless another valid form of payment is provided at the time.
- If payment is not made or if there is a chargeback or billing dispute, you understand that Dr. Lund is authorized to provide the necessary information to the credit card company about the scope of services provided to you as well as a copy of this signed form to obtain payment. Additionally, patients are responsible for the costs of collections if Dr. Lund must hire the services of a collections agency to obtain payment.
Cancellation, Rescheduling and Missed Appointment Policy:
- Consultations block out a large chunk of time in Dr. Lund’s schedule. Therefore, we request that you cancel/reschedule all appointments with at least 48 hours prior notice.
- For the initial 75 minute consultation, if you cancel with less than 48 hours’ notice or miss the appointment without notice (no-show), you will forfeit your deposit, and you will be required to pay the full amount of a new appointment if you decide to reschedule.
- For all other appointments, cancellations or rescheduling must be made with at least 48 hours prior notice. For cancellations made less than 48 hours before your scheduled appointment and No-Shows (Missed Appointments), you will be charged a $195 cancellation/no show fee.
- Late Arrival Policy: If you arrive late to your appointments, your appointment will end at the scheduled time, and you will be charged for the full length of the originally scheduled appointment.
Patient Communications:
In order to ensure the safety and confidentiality of your health information, Dr. Lund uses the HIPAA-compliant Electronic Medical Records System and secure Patient Portal.
- Please send all health-related messages to Dr. Lund through the Patient Portal. This ensures all messages are secure and retained in your medical record. In addition, through your account on the Patient Portal you can view your lab results and schedule follow-up appointments.
- Please do not e-mail health-related information and questions outside of the secure Patient Portal as e-mail can never be guaranteed to be confidential or secure.
- Patient portal messages are for quick clarifications and questions that do not require much discussion. They are not a substitute for an appointment with the doctor. If you have additional concerns and questions, please book an appointment.
- Dr. Lund will do her best to respond to Patient Portal messages within 2-3 business days, but response time is not guaranteed. Patient Portal messages are not for urgent needs.
- Dr. Lund does not provide urgent or emergency care. If you need urgent assistance, please go to urgent care. If you need emergency assistance, please call 911.
Ordering Supplements: To ensure patients have access to therapeutic-grade and high-quality supplements, Dr. Lund primarily recommends supplements through the online supplement dispensary platform Fullscript. When you purchase products from the Dr. Stephanie Lund Fullscript dispensary, Dr. Lund also receives a small commission. You do not pay more and may also receive a discount off the retail prices. You are under no obligation to purchase supplements from the Dr. Stephanie Lund Fullscript dispensary, and your decision as to where to purchase supplements and products will not affect your quality of care. Dr. Lund only recommends supplements as necessary and appropriate to the individual patient.
Lab Tests
- Dr. Lund generally orders labs through Rupa Health and other lab testing providers. Payment will be made directly by the patient to the lab.
- Insurance may cover some labs, depending on your individual plan, but we cannot guarantee what and how much your individual insurance plan may cover or what your out-of-pocket costs may be. It is the patient’s responsibility to understand her insurance coverage. To prevent the stress of unexpected bills, if you wish to use your insurance for lab testing, we recommend you contact your insurance company to have a good understanding of your lab benefits prior to completing any lab tests.
- With respect to labs ordered through Rupa Health, you can learn more about payment options on Rupa Health website: https://www.rupahealth.com/patients.
- If you have purchased a package where labs are included and you decide to run labs through your insurance, you will receive a credit to your account in the valued amount for those labs.
No Insurance or Medicare
- Dr. Lund is not contracted with any insurance carriers and is not a Medicare provider. Therefore, Dr. Lund cannot and does not accept or bill insurance or Medicare or submit claims for any services provided. All payment is due at the time of service.
- Upon request, we will provide you with a statement showing your payment and a coded superbill that you may submit to your insurance company (not Medicare) for possible reimbursement at an out-of-network rate depending on your individual plan. We cannot guarantee whether or how much your insurance company will reimburse you for your appointments. Payment reimbursement is subject to your insurance plan and we do not communicate with insurance companies. You are ultimately responsible for the cost of your care. For those with Medicare and HMO coverage, a claim cannot be submitted.
Medications and Prior Authorizations
- Insurance may cover some medications prescribed by Dr. Lund. Depending on your individual plan, you may choose to use your insurance rather than pay out of pocket for your medication. Should you choose to use your insurance and a prior authorization is required, a minimum fee of $50 per prescription will be charged to your credit card on file. This fee is for the time it takes to complete a prior authorization, which can be time intensive for your provider. The cost may vary depending on the amount of time and resources needed to complete the necessary paperwork.
